Will I pay the high-income surcharge (IRMAA)?

IRMAA is a cliff, not a slope, and it looks at your tax return from two years ago. Enter that income and see which band you land in and what it adds per month.

What this can't do: Based on the income you enter. Social Security decides using the return the IRS sent them.

Roughly your adjusted gross income plus any tax-exempt interest, from the return you filed in 2025. Not this year's income.

Two things nearly everyone gets wrong about IRMAA

It's a cliff. Cross a threshold by a single dollar and the surcharge applies to the whole year, at the full amount for that band. Income tax works in slices, where only the money above a line is taxed at the higher rate. IRMAA doesn't. That makes the last few thousand dollars of income near a threshold unusually expensive.

It's looking at an old tax return. Your 2026 surcharge comes from your 2024 return, because that's the most recent one the IRS has passed to Social Security. So a surcharge often lands in the first year of retirement, calculated on a final year of full-time salary. If that's you, the SSA-44 form above is the fix, and it's free.

The 2026 numbers in context are in 2026 Medicare costs, all on one page.

Nothing you type here leaves your device

This tool runs entirely inside your own browser. What you enter is never sent to us, never stored, and never shared. You can disconnect from the internet after the page loads and it will still work. That is deliberate: we tell people never to hand their Medicare number to a website, so this site had better not be one that asks.

Where these numbers come from

These are the 2026 amounts. CMS usually publishes the next year's figures in mid-November and SSA in October; after that, check the source links before relying on a result.
